Attention: ETTP Grandfathered Retirees:
IMPORTANT INFORMATION FOR EVERY GRANDFATHERED RETIREE! About a month ago, each retiree should have received an envelope in the mail from Mercer, the company who is managing the administration of our Health and Welfare Benefit Plans. The mailing was to notify us that we each need to designate our beneficiaries for our life insurance and 401(k) plans. If you have not recently done this, below is the information from the Mercer letter.
LIFE INSURANCE BENEFICIARIES — When the administration of our benefits was outsourced to Mercer Single Source 1 on February 1, 2014, the names of our life insurance beneficiaries apparently did not transfer. There are two ways to designate our beneficiaries on life insurance — via the Mercer SS1 website or by telephone. Be sure to have the name and social security number of each beneficiary before proceeding.
1. To enter the beneficiary information online, go to the Mercer SS1 Human Resource Benefits Advantage (HRBA) website – www.hrbenefitsadvantage.com
Once on the website, if you have not previously registered, click on “Register Here” and follow the directions. The Company code for participant registration is ETTP00 (i.e., E T T P Zero Zero).
Once you have established your User Name and Password, use the following path in HRBA to record your beneficiary information:
View/Enroll in Benefits > Make/Change Benefit Enrollment > Beneficiary Change (drop down box) > Continue Enrollment > Add/Change/Remove Life Enrollment > Add/Change Enrollment (Enter Beneficiary Information) > Check the “Authorize” box > Save for Approval. Select “Confirmation” if you would like to view a statement of your benefits.
2. To designate life insurance beneficiaries by phone, call Mercer SS1 directly at 888-890-5631.
401(k) PLAN BENEFICIARIES — Previous beneficiary designations did not transfer from JP Morgan to Fidelity Investments for those retirees who continue to have a balance in the 401(k) plan. Please contact Fidelity at 1-800-835-5095 or online at http://www.netbenefits.com and take the appropriate action to designate your beneficiary information for your plan account.
